Spine Surgery in India: When Is It Actually Necessary?
Many spine surgeries in India are performed unnecessarily. Here's the consensus on what conditions truly require surgery and what can be treated conservatively.
The Surgery Conversation in India
India performs an estimated 400,000–600,000 spine surgeries annually, with growth rates of 15–20% per year driven partly by growing surgical capability and partly by a healthcare environment that can incentivise procedures. Understanding when surgery is and isn't indicated helps patients make informed decisions.
Absolute Surgical Indications (Surgery Is Necessary)
These are conditions where delaying surgery can cause permanent neurological damage:
Cauda Equina Syndrome: Compression of the cauda equina nerve bundle causing bowel/bladder dysfunction, saddle anaesthesia, and bilateral leg weakness. Surgical emergency — delay beyond 24–48 hours causes permanent deficits.
Progressive Neurological Deficit: Increasing weakness in arms or legs due to spinal cord or nerve root compression. Surgery within days is indicated to prevent permanent deficit.
Myelopathy with Functional Decline: Spinal cord compression causing deteriorating hand function, gait disturbance, or balance problems that are progressing despite conservative treatment.
Spinal Tumour or Infection: Structural instability from pathological processes requires urgent surgical stabilisation.
Relative Indications (Surgery Is an Option, Not Mandatory)
Persistent radiculopathy (sciatica) not improving after 6–12 weeks of conservative treatment
Spinal stenosis with claudication significantly limiting daily activity
Spondylolisthesis with mechanical instability and pain unresponsive to physiotherapy
What Does NOT Need Surgery (But Often Gets It)
Disc bulge or herniation without neurological deficit — the vast majority resolve with conservative care within 6–12 weeks
Degenerative disc disease without instability or nerve compression
Non-specific low back pain — surgery has no better outcomes than conservative management for this
MRI findings of disc changes without corresponding clinical symptoms — treat the patient, not the scan
Questions to Ask Before Agreeing to Spine Surgery
What is the specific structural cause being treated?
What is the expected surgical outcome and timeline?
What happens if I wait 6–12 weeks of quality conservative treatment first?
What are the specific risks of this procedure (infection, hardware failure, adjacent segment disease)?
